I acknowledge that my attendance and/or participation in the food tour hosted by Chicago Food Walks LLC (the “Tour”) may involve the risk of physical injury or harm resulting from the consumption of certain foods and/or beverages. I have been advised that allergenic components may be present in foods and/or beverages consumed during the Event. I HEREBY VOLUNTARILY ASSUME THE RISKS OF ATTENDING AND/OR PARTICIPATING IN ANY ACTIVITY AT OR ASSOCIATED WITH THE TOUR INCLUDING THOSE RISKS ASSOCIATED WITH THE CONSUMPTION OF FOOD AND/OR BEVERAGES, WHICH MAY INCLUDE BUT ARE NOT LIMITED TO FOOD SICKNESS, ALLERGIC REACTION, DISABILITY OR DEATH.
I further agree to the following:
- I am free to choose which foods and/or beverages, if any, I consume.
- I agree to disclose any and all food allergies to Rebecca Wheeler prior to Tour commencement. I will not consume food to which I am allergic.
- I agree not to consume alcohol during the tour if I am less than 21 years of age or do not have valid government issued identification in my possession during the Tour.
- I represent that I am in good health and capable of walking long distances outdoors. I am encouraged to wear proper clothing, use sunscreen, and stay hydrated throughout the Tour. I am sufficiently self-aware to stop physical activity before I become ill or injured.
- If while on the tour I begin to feel faint or nauseous, I will immediately notify the tour guide and seek medical attention if necessary.
- I am aware that streets along the tour route are open to vehicular traffic and I will obey all traffic laws/regulations.
In consideration for my participation in the Tour, I hereby state the following:
I WAIVE, RELEASE, AND DISCHARGE CHICAGO FOOD WALKS LLC, and/or any of her agents (the “Parties”), from any and all claims, losses, or liabilities for personal injury, partial or permanent disability, property damage, medical or hospital bills, theft, or damage of any kind, including economic losses, which may arise by reason of my participation and/or presence at the Tour. To the extent possible by law, I hereby fully waive, release, and discharge the Parties notwithstanding their own negligent acts or omissions or the negligent acts or omissions of others.
I agree that I will not bring any action or claim against any of the Parties hereby released for any reason associated with the Tour.
I INDEMNIFY AND HOLD HARMLESS the Parties hereby released from any and all claims made or liabilities assessed against them as a result of my participation in the Tour.
I HAVE READ THIS DOCUMENT AND I UNDERSTAND ITS CONTENTS. I ACKNOWLEDGE THE POSSIBLE HAZARDS OF ATTENDING AND/OR PARTICIPATING IN THE TOUR AND I AM FULLY AND VOLUNTARILY WAIVING ALL LIABILITY THAT MAY ARISE BY REASON OF THIS PARTICIPATION AFTER FULL CONSIDERATION OF THE RIGHTS I AM WAIVING. THIS WAIVER IS IN CONSIDERATION OF MY BEING ALLOWED TO PARTICIPATE IN THE EVENT.
